Step 2:

a. Enclose all of the following in an envelope: The original completed Money Back Guarantee Form.

b. The original store register receipt with a specially-marked Depend® Fit-Flex® (Maximum Absorbency), Depend® Night Defense® product, Depend® Protection Plus Ultimate, Depend® Real Fit, or Silhouette® product purchased between 1/1/22 – 12/31/24. Please circle product purchased and date on store receipt.

c. The original UPC Code from the package. UPC Code must match offer packaging.
